Developmental evaluation to improve provisions of intermediate care in Bangkok health zone 5

Abstract

Background: Post-acute care or intermediate care is crucial to restore function of stroke patients. However, many stroke patients in Thailand didn’t received this important service and turned to be disable persons. The Ministry of Public Health, Thailand has realized this problem of inaccessibility to intermediate care and has introduced a policy (a service plan for intermediate care) to increase services since 2020 but the implementation of the policy hasn’t been reaching its high potential, especially in the capital of Thailand, Bangkok as developments of health service systems have been guided toward tertiary care. Under the new governor of Bangkok, Dr. Chadchart Sittipunt, primary care has been emphasized more to strengthen the network of primary care for Bangkok metropolitan, or a primary care reform. Access to intermediate care for stroke patients including admission to intermediate ward, attending outpatient rehabilitation care or receiving rehabilitation at home would be one of indicators to evaluate the success of this primary care reform in Bangkok.

Methods: Developmental evaluation would be applied to both monitor and prompt feedback in order to improve access and quality of intermediate care, especially home rehabilitation services. This study will apply mixed methods to conduct the evaluation.

Results: At Bangkok Zone 5, the Klang Hospital, the leading hospital in the zone, established intermediate ward in August 2023, and providing intensive rehabilitation to 31 patients within 4 months. Fourteen of these patients had full barthel index on discharge days.

Conclusion: The implementation of the primary care reform showed positive effects in providing vital services to patients who were in need and further evaluation will be conduct to monitor access to home rehabilitation to ensure the success of primary care reform in Bangkok.
