Evolution of management of diabetes mellitus
Abstract
Diabetes is an ancient disease. Over centuries extreme diets and herbal remedies were used to treat diabetes symptoms. The discovery of insulin in 1921 transformed the landscape of diabetes treatment. This was followed by the consecutive discovery of several new therapies which improved management of glycemia and hence living of innumerable patients. The DCCT and the UKPDS trials demonstrated that tight glucose control reduced the microvascular complications of diabetes, but had marginal effects on cardiovascular diseases. In 2008, the FDA directed that all new diabetes medications should demonstrate cardiovascular safety. FDA approval the combined GIP and GLP-1RA and SGLT2i drugs for the treatment of adults with Type 2 Diabetes (T2DM), have transformed the horizone of diabetes treatment through their robust cardiorenal benefit. Clinicians are flowing the contemporary guidelines for diabetes mangament. Developments in technology like smart pen, continuous glucose monitoring systems (CGM), insulin pumps has advanced monitoring and treatment. Precision medicne telemedicine, artificial intelligence are widening scopes. Scientist are still in quest of physiological on demand delivery of insulin in circulation. Progress continues in stem cell use and islet transplantation. Today Type 2 Diabetes is preventable adopting the fundamentals of diet and weight adjustment; Long-term remission is possible with metabolic surgery. (WHO) predicts an approximate of 537 million adults (ages 20–79 years) are living with diabetes today, and this number is to rise to 643 million by 2030 and 783 million by 20451. UNO have introduced “World diabetes day” aiming to achieve satisfactory control of DM worldwide by enhancing awareness and swapping the attitude of the chronic sufferer.
Biography
Khwaja Nazim Uddin completed MBBS from Dhaka Medical college. Earned (FCPS) from Bangladesh College of Physicians and Surgeon (BCPS). He was awarded FACP and FRCP in 2007. He served as professor of medicine and senior consultant BIRDEM. Bangladesh Institute of Research and Rehabilitaton in Diabetes Endocrine and Metabolic Disorders- an institute dedicated for service of the diabetic population. He served as executive editor of Birdem medical journal(birdemmedj). He is a faculty of medicine of BCPS and editorial board member of Journal of BCPS. He is an Examiner as well as reviewer of thesis, dissertation. He has over 82 publications.
